CEU Approval

The Academy of Pelvic Health Physical Therapy does not individually apply for course approval numbers to each state. It is up to each individual to submit for CEU’s within their respective state, however we are here to help you obtain and provide any necessary documentation you need for your CEU application. Our course participants have never had trouble getting a course approved for continuing education credits (CEUs) once they applied with their respective state's board.

Not sure what documents you need to provide to your state board?  All State Licensure Boards are listed on the FSBPT: The Federation of State Boards of Physical Therapy website where you can find their CEU requirements.   2. Course Objectives
  3. Course Time Ordered Agenda
  4. Faculty CVs / Resumes

  5. Evaluation Survey (Live Courses ) | Evaluation Survey (On-Demand Courses)
  6. Certificate of Completion (visit your Learning Center)
  7. Bibliography with up to 10 sources from the last 10 years (visit your Lecture manual reference pages)
